TO NIS-1 SIGNED SEPTEMBER 26, 1987 Introduction Inservice examinations of Class 1, Class 2, and Class 3 components, piping and component supports were conducted at North Anna Power Station Unit 1 from December 25, 1985 to June 28, 1987.

Examination procedures were approved prior to the examinations.

Certification documents relative to personnel, equipment, and materials were reviewed and determined to be satisfactory.

Inspections, witnessing, and surveillance of the examinations and related activities were conducted by personnel from the Hartford Steam Boiler Inspection and Insurance Company, One State

Limitations Some of the arrangements and details of the piping system and f

components were designed and fabricated before the access and examination requirements of Section XI of the 1974 Code could be applied; consequently some examinations are limited or not practical due to geometric configuration or accessibility.

Examinations Examinations were conducted to review as much of the examination zones as was practical within geometric, metallurgical and physical I

limitations. When the required ultrasonic examination volume or area could not be examined 100%,

the examination was considered to be a

partial (PAR) and so noted on the examination report. Generally PARS are noted at fitting to fitting assemblies and where integrally welded supports, lugs or hangers, etc., preclude access to some part of the examination area.

A)86-025 - Replaced the bonnet and disc on "B" Reactor Coolant Loop Bypass Valve 1-RC-MOV-1586.

The valve was repaired to eliminate packing leakage.

A summary of the indications and their dispositions follows:

A) Visual mination of the inter or of safety injection valve a

,,RA-1-6200 Item 11 shown on ISO VRA-1-4105) revealed two indications on the body flange gasket surfaces.

The indications were evaluated by Virginia Electric & Power Company Engineering to be minor and could be removed by honing.

The indications were removed by honing and found to be acceptable upon reexamination.

B) Visual examination of the interior of "B" Reactor Coolant Loop Bypass Valve 1-RC-MOV-1586 revealed the following indications:

1. Valve body interior - minor scratches and scrapes
2. Valve bonnet - hole drilled in bonnet
3. Yoke spacer ring - badly eroded and corroded
4. Valve disc - abnormal wear on seating surface 5.

Yoke lock rings - heavily eroded and corroded 6.

Nuts and bolts - severely corroded The conditions noted were corrected under repair and replacement program 86-025.

The valve was found to be acceptable upon reexamination.
