| description = The inspectors identified an unresolved item regarding 10 CFR 50.59 Changes, Tests, and Experiments. The licensee installed tanks containing sodium hypochlorite and Hydroxyethylidenediphosphonic acid (HEDP, a strong acid) and may not have accounted for conditions for an accident of a different type than any previously evaluated in the Updated Final Safety Analysis Report (UFSAR). On July, 15, 2011, the licensee restricted access to the crib house due to a combined spill of sodium hypochlorite and HEPD. The mixture of the two chemicals produced chlorine gas. Based on initial assessment of the event and meeting the Emergency Action Level (EAL) threshold criteria an Alert, HA7, was declared at 10:16 a.m. The Alert was terminated at 3:20 p.m. on July 15, 2011, when actions to ventilate the crib house were completed and all areas were verified to be clear. The inspectors were in the process of reviewing two modifications in regard to 10 CFR 50.59 which may not have accounted for conditions for an accident of a different type than any previously evaluated in the UFSAR. The first was the addition of the chemical storage tanks and the second was the removal of the control room ventilation Toxic Gas Analyzer from service.
