Using Manual Chapter 0609, Appendix F, Fire Protection Significance Determination Process, the inspectors conducted a Phase I significance determination process screening utilizing Attachment 1 of Appendix F. In accordance with the Phase I screening criteria, this finding was assigned a category of Fire Confinement, and a category of Low Degradation Rating, because the degraded fire barrier door did not affect more than one fire area. Using the qualitative screening criteria of Appendix F, this finding was determined to have very low safety significance because more than one fire area was not affected, and because the other emergency diesel generator would be credited to safely shutdown the plant. This finding has a crosscutting aspect in the area of problem identification and resolution associated with the corrective action program because the licensee did not ensure a low threshold for identifying issues and ensure that conditions adverse to quality were identified completely, accurately, and in a timely manner commensurate with their safety significance
