RENEWED FACILITY OPERATING LICENSE NPF-30 OWNERS ACTIVITY REPORTS (OAR-i FORMS) FOR CYCLE/REFUEL 24 Please find enclosed the Owners Activity Reports (OAR-i forms) for Cycle 24 and Refuel 24 activities at Callaway Plant. The reports are submitted pursuant to the provisions ofthe applicable revisions ofASME Boiler and Pressure Vessel Code Case N-532, Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission.

The OAR-f forms enclosed are associated with the following Cycle 24 and Refuel 24 activities at Callaway Plant:

1.

Inservice Inspection (151) and Repair/Replacement activities for the second period of the fourth 151 Program interval, and 2.

Containment Inservice Inspection pursuant to ASME Section XI, subsection IWE for the first period of the third IWE Program interval.

Each enclosed OAR-i form includes a listing of items with identified flaws or relevant conditions that required evaluation for continued service (Table 1) and an abstract of repair/replacement activities required for continued service (Table 2).

In accordance with Revision 1 ofthe Fourth Interval 151 Program Plan, the OAR-i form for the 151 and Repair/Replacement activities is submitted in accordance with Code Case N-532-5. Similarly, in accordance with Revision 4. 1 ofthe IWE Program Plan, the OAR-i form for the Containment Inservice Inspection activities is submitted in accordance with Code Case N-532-5.

CALLAWAY NUCLEAR PLANT TABLE I ITEMS WITH FLAWS OR RELEVANT CONDITIONS THAT REQUIRED EVALUATION FOR CONTINUED SERVICE Examination Category Item Description Evaluation Description and Item Number E-A, E.1.I 1 Incore Sump Liner CR 202005276 While performing Job 17514095.500, IWE inspection ofthe lncore Pit, Degraded coatings were identified. After the first inspection on 10/7/20, QC notified R/C Helpers to perform additional cleaning of the pit and took the job to Stoppedw. QC reinspected on 10/8/20 after cleaning was performed and identified multiple areas of bubbled coatings. A paint scraper was used to expose the liner plate on all of these locations, so the WE exam could be completed. All areas identified had sound metal exposed and for the purpose of the IWE General Inspection, ESP ZZ-0I016 Section 3 and the Incore Pit, Area 31 was accepted. The Incore Pit will have coatings repaired in Refuel 25 to avoid degradation that would cause extensive structural repairs.
