17, 1987, in Inspection Report 50-461/87011.

this letter for the details of the Illinois Power, Company (IP) response to j

the specific examples cited.

The generic concern of this violation is lack of procedural adherence.

Illinois Power management has reinforced the necessity _of J

strict procedural adherence. The three examples cited in the violation were all in the area of operations.

IP has two programs which will J

specifically monitor procedural adherence in this area.

IP has i

I implemented an Operations Monitoring Program. This program places observers in the Control Room for the express purpose of observing the operating crew and provides feedback to the Plant Manager and operators.

In addition, a Performance Monitoring Program dedicated to assessing operator performance, including procedural compliance, while performing startup and shutdown evolutions will be conducted during Test Condition 2.

Attachment A Illinois Power Company Clinton Power Station The Notice of Violation states in part:

1.

"On February 27, 1987, day shift and swing shift Control Room Operators failed to convey the status of out-of-specification differential pressure readings they had recorded during the conduct of surveillance CPS No. 9000.01D001, Control Room Operator Surveillance Log - Mode 1, 2, 3 Data Sheet.

On March 28, 1987, during exercising of the Traversing In-Core Probes per CPS No. 3322.01, the on-duty shift supervisor made a decision to allow the work to continue after finding that the procedure did not l

provide sufficient instruction in step 8.1.2.12 to allow the operator to get to the next step without additional manipulation of the equipment. This was done without making a change to the procedure to insert the additional step that was required to be taken.

3.

On March 30, 1987, personnel performing surveillance procedure CPS No. 9030.01C035, Revision 20, failed to implement the instructions contained in Plant Manager Standing Order No. 30.

This failure resulted in an unnecessary ESF actuation when the Reactor Core Isolation Cooling System isolated."

Corrective Actions Taken and Results Achieved 1.

The operators involved were disciplined for their lack of.

action. The remaining shift operators were apprised of the situation and reminded of the appropriate actions.

2.

Procedure Number 3322.01, Traversing In-Core Probes, has been revised to provide sufficiently detailed instructions to properly operate the equipment.

3.

The personnel involved have been counseled on the performance of the impact matrix in accordance with Standing Order Number 30.

Procedure Number 9030.01 has been revised to provide for removal of the system from service when performing the surveillance test.

II.

Corrective Actions Taken to Prevent Recurrence 1.

No further actions are necessary.

2.

Nuclear Training vill review and revise as necessary the Licensed Operator Training Program to ensure that the lesson plan for operation of the Transversing In-Core Probe System is correct.

A review is being conducted by Nuclear Station Engineering (NSED) to identify all instruments which trip or isolate on a one-out-of-one icgic. Upon completion of_this by NSED, Operations will review the affected surveillance procedures and revise those procedures which may cause an ESF actuation.
