On March 9,1984, we s'erbally notified your Mr. D. Hunnicutt of a potentially reportable item involving curtain fire dampers which were installed without a specified gap between the moveable curtain and its structure. Specifically, j

the gap is required to allow for thermal expansion of. the damper during service conditions, and allow it to function without binding. This item was determined to be reportable on July 18, 1984 and was discussed;with your Messrs. Collins, Hunnicutt, Bangart and Cummins, as well-as, Mr. Wessmon of NRR. We previously reported this i
: sue by our letter logged TXX-4257, dated August 13, 1984, in which we described the corrective actions being taken for this deficiency. This item was subsequently identified by our last report logged TXX-6435, dated May 13, 1987, as an item for which the corrective actions require revision. This supplemental report is submitted to provide the current status of our corrective actions.

The Deviations, referenced in the Attachment to TXX-4257 were' submitted in the l Fall of 1984, and were subsequently accepted per SSER-12.

A letter was issued in accordance with 10CFR part 21 by the manufacturer of-the existing dampers (Ruskin) stating that closure under airflow could not be

{ assured. The closure problem combined with the aforementioned installation configuration irregularities have combined to make the problems associated with dampers twofold.

After extensive evaluation, we have concluded that the prudent, most expedient solution is to replace all essential fire dampers with new dampers which meet VL requirements and will close under airflow. Design Change Authorization-(DCA) 51454 Rev.1 has been issued identifying all-fire dampers which require replacement in the hVAC systems. Purchase orders have been issued to procure the new dampers and obtain field technical services from the vendor. These l

services will ensure correct measurement, fabrication and installation of the L new dampers.

Dampers which have been determined to be nonessential will be abandoned in accordance with DCA 25689 Rev. 2. The installation of these fire dampers is presently scheduled for completion by December 31, 1987.

Our next report on this issue will be submitted no later than January 31, 1988.
