COMANCHE PEAK STEAM ELECTRIC STATION (CPSES)

DOCKET NOS. 50-445 AND 50-446 CABLE TRAY SPLICES / CONNECTIONS SDAR: CP-86-52 (INTERIM REPORT)

Gentlemen:

On July 25, 1986, we verbally notified your Mr. I. Barnes of a deficiency involving splice / connections used for cable trays in Units 1 and 2. We have concluded that this deficiency is reportable under the provisions of 10CFR50.55(e) and the required information is as follows. We requested and received an extension until May 8, 1987, in providing our response in a telephone conversation with Mr. I. Barnes on April 28, 1987.

Description of the Problem During as-built activities on the cable tray hangers for Unit 2, a deficiency involving incorrect installation of splice / connections was identified.

Specifically, the splice / connections used for some tray types and sizes were not installed in accordance with specified splice patterns as defined by the cable tray manufacturer or approved CPSES design or design change documents. The QC Inspection requirements delineated by procedure QI-QP-11.3-24, " Class 1E Cable Tray Raceway Inspections," did not include verification of the splice / connector configurations or dimensions.

Engineering evaluation of the as-built information for both units has indicated that a potential failure could occur in "Z" type splices. These splices have been categorized as potential failures, because these connections would be expected to fail when subjected to seismic loading conditions.

Splice configurations for both units which require further engineering evaluation have been selected for testing. The test specification has been completed and the test is expected to begin by May 4, 1987. Upon completion of testing, an engineering evaluation of the results will be performed and additional recommendations made for corrective actions if required.

Safety Imolication Failure of cable trays to perform as intended by engineering design could result in an inability of associated safety-related electrical systems to perform as required for safe operation / shutdown during a seismic event.

- TXX-6415 April 29, 1987 Pag] 2 of 2 Corrective Actions The following measures have been or will be implemented to prevent the misuse of splice plates in Units 1 and 2.

1. Design Change Authorizations (DCA's) have been issued to provide criteria per specifications ES-100, " Electrical Erection," via DCA 31431 and ES-19, " Cable Tray Specifications," via DCA 34029. Previously generated splice plate DCA's have been voided to prevent use.
2. Following the issuance of these DCA's, electrical construction procedure ECP-10, " Cable Tray and Hanger Installation Unit 1," and ECP-10A, " Cable Tray and Hanger Installation Unit 2," will be revised to indicate acceptable splice plate configurations.
3. NCR's will be generated to identify specific occurrences of the use of "Z" type splices and document specific measures.
4. Rework on future installations will be accomplished using approved procedures and specific DCA's against unique tray sections.
5. QC Inspections Procedure QI-QP-11.3-24, " Class 1E Cable Tray Raceway Inspections," will be revised to include inspection of splice plate configuration and dimension criteria.

Based on the projected schedule, our next report will be issued no later than August 3, 1987.
