DOCKET NOS. 50-445 AND 50-446 WELDED ATTACHMENTS TO EMBEDDED STRIP PLATES SDAR: CP-86-44 (INTERIM REPORT)

Dear Mr. Johnson:

On June 5,1986, we verbally notified your Mr. I. Barnes of a deficiency involving the design / construction requirements for minimum separation of welded attachments to embedded strip plates which were not properly reflected in inspection procedures.

This is an interim report of a potentially reportable item.under the provisions of 10CFR50.55(e). We have submitted interim reports logged TXX-4892 and TXX-4978, dated July 3,1986, and August 22, 1986, respectively.

During the course of this evaluation one additional related concern was noted.

Existing quality control instructions did not delineate criteria for the separation between loaded embedded plates and Hilti anchor bolts. Therefore, the scope of this evaluation has been expanded to encompass this issue.

To prevent recurrence of this condition, the following action has been formulated. Specification 2323-SS-30, " Structural Embedments," has been revised and Civil Engineering's instruction CEI-20 is being revised to clarify concrete anchor spacing criteria. Construction procedure CCP-45, " Permanent and Temporary Attachments to Embedded Weld Plates," has been revised to clarify installation criteria and construction attributes.

Quality Control Procedure QI-QP-11.14-13, " Separation Verification of Attachments to loaded Embedded Plates," has been developed to address the inspection of attachments to embedded plates and spacing requirements for welded attachments, loaded Hilti bolts, and Richmond inserts. This procedure will also address the inspection of previous and in-process installations.

Due to the revised scope of this issue, the identification (through field verification and subsequent evaluation) of installations in Unit 1 involving the spacing of anchors / attachments is scheduled for completion by January 16, 1987.

In Unit 2, the Wall Embedments and Beams (WEB) program will provide identification and analysis of all embedded plate attachments.

A plan for the reinspection of existing Unit 2 embedded plate attachments is in development.
