DOCKET NOS. 50-445 AND 50-446 WELDED ATTACHMENTS TO EMBEDDED STRIP PLATES SDAR: CP-86-44 (INTERIM REPORT)

Dear Mr. Johnson:

On June 5,1986, we verbally notified your Mr. I. Barnes of a deficiency involving the design / construction requirements for minimum separation of welded attachments to embedded strip plates not being properly reflected in inspection procedures. This is an interim report of a potentially reportable item under the provisions of 10CFR50.55(e). We have submitted an interim report logged TXX-4892, dated July 3, 1986.

A preliminary evaluation has been conducted in Unit 1 by Engineering to determine the approximate scope of embedded plate welded attachment spacing violations. It was determined out of approximately 800 embedded plate attachments, 113 violations exist. TUGC0 QC will perform a backfit inspection to determine the as-built condition on all embedded plates for Unit 1. The inspection results will identify and document all embedded plates with attachment violations. The Structural Embedment Group (SEG) will then evaluate the violations for acceptability. In Unit 2 the Wall Embedments and Beams (WEB) Program will track and analyze all embedded plate attachments.

We are continuing our evaluation and anticipate submitting our next report by October 20, 1986.
