COMANCHE PEAK STEAM ELECTRIC STATION DOCKET NOS. 50-445 AND 50-446 ELECTRICAL PENETRATION ASSEMBLIES SDAR: CP-86-10 (INTERIM REPORT)
Dear Mr. Johnson:
On February 14, 1986, we verbally notified your Mr. T. F. Westerman of a deficiency involving electrical penetration assemblies (EPA's) supplied by Bunker Ramo which may not be in accordance with design requirements. We have submitted an interim report TXX-4733, dated March 17, 1986.
We have concluded that this deficiency is reportable under the provisions of 10CFR50.55(e). We anticipate evaluation of all corrective action and submittal of our final report by June 11, 1986.

't ELECTRICAL PENETRATION ASSEMBLIES DESCRIPTION During construction inspection activities, electrical " pigtail" splices were observed in two EPA (Electrical Penetration Assembly) modules which were not staggered as specified in the vendor requirements. The nonconformance report issued to document the conditions was further expanded into a project Corrective Action Request (CAR), as a result of the indeterminate scope of the issue.
The function of an EPA is to seal the containment structure at the point electrical conductors exit the building.
Investigations by site quality and engineering groups has resulted in identification of the following deficiencies involving EPA modules supplied by Bunker Rama:
a)
" Pigtail" splices were not performed in accordance with vendor fabrication documents, b)
Rework of heat shrink cable splice jackets has not be performed as required.
c)
The application of heat shrink cable splice jackets is inadequate.
d)
Exposed conductors have been observed at splices.
e)
The vendor QA program has failed to provide a method to advise the project of required correction.
f)
The QA program has failed to effectively identify items and activities requiring correction.
In order to fully investigation the circumstances resulting in the EPA deficiencies, a project Task Team has been formed.
The conclusions of the Task Team are anticipated in mid-May 1986.
Please note this issue has been specifically reviewed by the NRC per Inspection Reports 445/86-04 and 446/86-03.
The concern is applicable to Units 1 and 2.
