On September 13, 1984 we verbally notified your Mr. D. Hunnicutt of a deficiency regarding ventilation exhaust dampers that have been observed to be designed /
installed to fail closed which would divert radioactive releases to rooms in which operator action is required. We have submitted an interim report logged TXX-4333 dated October 12, 1984.
We are continuing our evaluation and anticipate crepletion by December 21, 1984.
