In accordance with 10 CFR 50.55(e), we are submitting the attached report of actions taken to correct the identified deficiency regarding certain Borg Warner valves which may be af fected if installed between 22-1/20 and 157-1/20 of vertical. We previously made a verbal report to Mr. L. Martin on January 22, 1982, with an interim report logged TXX-3479 and dated February 19, 1982.

W TXX-3495 March 18, 1982 ATTACHMENT INSTALLED BORG WARNER VALVES DESCRIPTION OF THE DEFICIENCY A supplier of power operated relief valves -- Borg Warner Nuclear Valve Division -- advised that the operability of twelve valves shipped prior to May,1979, can be affected when installed between 22-1/20 and 157-1/20 of vertical.
Site engineering has determined the following valves were a f fect,d:

Safety Implications If the valves fail in the full open or partially open position, the pump's suction will remain aligned to the Refueling Water Storage Tank (RWST) as well as other valves opening to align the pump's suction to the Containment Sumps. The RWST would be drained, but due to the suction head available from the sumps, the pumps will continue to operate normally.
During plant operation it is highly unlikely that the valves might fail in the closed position.
If either valve was inadvertently closed and was not able to be reopened, the plant would be required to shutdown per CPSES operations technical specifications.
Note: The plant can be brought to a safe shutdown condition with one of the two redundant Containment Spray Trains operable.
Corrective Actions a.
The gates for the four valves identified above have been or will be returned to the manufacturer for rework.
b.
A manufacturers representative will arrive on site and determine whether or not the damaged gate guide can be repaired in place.
If so, he will supervise the repair.
If not, the valve will be removed and returned to the manufacturer for repair.

Two of the four gates were shipped to Borg Warner on February 19, 1982 for rework.
Upon return to CPSES, the remaining two valves will be shipped to Borg Warner for rework.
b.
The gate guide hinges will be repaired upon completion of the flushing operation which is currently scheduled no later than March 31, 1982.
