Dear Mr. Madsen:
On February 17, 1981 in accordance with 10 CFR 50.55(e), we verbally reported to your Mr. R. G. Taylor a deficiency regarding a Westinghause valve that failed to completely close under pre-operational test conditions.
We h.ve submitted interim reports logged TXX-3288, TXX-3322, and TXX-3396 dated March 13, May 21, and August 31, 1981, respectively. These reports contained information on the affected three and four inch valves. Our lates+. report expanded the sec'e of the deficiency to include 6, 8, 10, 12, 14,16 and 18 inch Westinghouse valves.
We are submitting the attached report which will complete our reporting requirements under 10 CFR 50.55(e).

Corrective Actions Westinghouse has issued Field Change Notices 10515 and 10537 which detail the corrective actions to be implemented for all Westinghouse valves identified under the scope of this deficiency.
The modifications are scheduled to begin October 5,1981 and are expected to be complete no later than October 30, 1981.
t TXX-3409 September 28, 1981 ATTACHMENT WESTI!!GHOUSE GATE VALVES Please note that valves 8821A and B, Mc#14GM87 are used only to separate the hot leg safety injection from the cold leg safety injection and not used to isolate the normal and emergency flow paths in the Chemical and Volume Control System (CVCS) as previously stated in cur TXX-3288.
