The following information was provided by … The following information was provided by the Texas Department of Health (the Department) via email:</br>On July 19, 2025, the Department was notified by the licensee of a gauge shutter mechanism failure. A Vega gauge model SH-F2, containing a 300 millicurie cesium-137 sealed source, was reported to be stuck in the open position. Open is the normal operating position. The issue was discovered on July 18, 2025, and the licensee confirmed there is no risk of additional radiation exposure to the public or workers due to this on/off mechanism failure.</br>Additional information will be provided in accordance with SA-300 reporting requirements.</br>Texas incident number: 10210</br>NMED number: TX250035ident number: 10210
