The following information was provided by The following information was provided by the South Carolina Department of Environmental Services (the Department) via phone and email: </br>The licensee informed the Department on January 3, 2025, a portable moisture density device was hit by a piece of construction equipment which caused the device to be disabled or fail to function as designed. The licensee reported that the source rod was broken, but was found by performing a survey of the area. The licensee reported that the source rod was shielded back in the portable moisture density device, packaged, and transported back to the licensee's facility to await disposal. The licensee reported that no other damage to the portable moisture density device was observed. The portable moisture density device is a Humboldt 5001 gauging device (serial number 3521) housing approximately 10 millicuries of cesium-137 and 40 millicuries of americium-241:beryllium. The licensee did not report any overexposures. The licensee did not call the Department on January 3, 2025. The Department was made aware of the event on January 6, 2025. The licensee's representatives account of the event was consistent with the initial report. A 30-day written report was received on January 29, 2025. Details of the 30-day written report were consistent with the details obtained from the licensee during the on-site visit and interviews. A leak test record of the portable moisture density device (serial number 3521) dated January 17, 2025, indicated the leak test results of less than 0.005 microcuries. The portable moisture density device (serial number 3521) was transferred to the manufacturer for repair or disposal on January 17, 2025. As a result of this event the licensee will review emergency and operating procedures with all portable moisture density device users.
