The following information was provided by … The following information was provided by the Mississippi State Department of Health (MSDH) via email:</br>The licensee, Tronox, LLC in Hamilton, Mississippi (Monroe County) reported a fixed gauge (Ohmart SHF-2-30 Serial Number 5319GK, 200 mCi Cs-137) (with a) stuck shutter to Mississippi State Department of Health on late Friday afternoon (9/23/22). It was discovered (on) 9/22/22 at 1557 CDT.</br>The failure was identified during calibration activities being performed by a BBP representative who is an authorized Ohmart representative.</br>The source shutter failure appears to be caused by age and corrosion. No exposure has been received by Tronox personnel and the unit did not leave its normal operating position. A certified Ohmart representative will replace the shutter in place.</br>MSDH has asked for a timeline on the replacement, a current survey reading at the restricted area boundary, and what additional precautions have been taken to assure no unnecessary exposures.</br>Mississippi Incident Number: MS-220004es.
