The following report was received by the O … The following report was received by the Ohio Bureau of Radiation Protection via email:</br>The licensee reported a Varian Varisource 200 HDR unit failed to indicate the source was fully retracted at the conclusion of patient treatment. The licensee initiated emergency procedures and entered the room. Surveys confirmed the source had fully retracted into the unit and the patient did not receive any additional dose beyond what was prescribed. The unit console has been locked and placed out of service until the manufacturer can inspect and repair the device.</br>Ohio Report Number: OH210008</br>A Medical Event may indicate potential problems in a medical facility's use of radioactive materials.
