The following information was received froThe following information was received from the State of Texas via email:</br>On June 2, 2017, the Agency (Texas Department of State Health Services) was notified by the licensee that one of its radiography crews was unable to retract a 92.5 curie iridium - 192 source into a QSA 880D exposure device. The radiographers had performed several exposures. To perform the radiography, one end of a flexible guide tube was connected to the exposure device and the other end was connected to a stiff guide tube. After completing an exposure, the source hung up in the stiff guide tube and would not retract into the exposure device. The radiographers contacted their management who responded to the site. The stiff guide tube was examined and was found to have several indentations in it that appeared to be preventing the source from passing through the tube. The recovery team straightened out the guide tubes and was able to retract the source into the fully shielded position. The licensee stated the guide tube was not damaged on this date. The licensee has removed the stiff guide tube from service. Additional information will be provided as it is received in accordance with SA 300.</br>Texas Incident No.: I-9488ce with SA 300.
