The following information was obtained froThe following information was obtained from the state of Louisiana via email:</br>On Friday, March 10, 2017, TEAM Industrial Services, Inc., in Borger, TX expected to receive two Ir-192 radiography camera sources from Source Production & Equipment Co., Inc., in St. Rose, LA. Only one source arrived at TEAM Industrial. The transportation company began a search for the missing source and discovered that the source had been shipped to Portland, OR. The source, 105 Ci of Ir-192 (as of 3/13/17), is enroute to TEAM Industrial in Texas and expected to be there by Thursday 3/16/17. The source was a G-60 source, serial number YC0801, in a SPEC-150, serial number 137.</br>Louisiana Event Report ID No.: LA170003</br>*** UPDATE ON 3/21/17 FROM JAMES PATE TO NESTOR MAKRIS ***</br>The following update was received via email:</br>I am updating a notification in writing. The confirmation number was 52606 from the HOO. Louisiana Event Report ID Number is LA170003. The transportation misplaced source was located and was received by TEAM Industrial on Friday 03/17/2017.</br>NMED, please closed out this event. The source was received at the designated receiving location on Friday 03/17/2017.</br>Notified R4DO (Drake), ILTAB DO (Davis), and NMSS Events Notification.</br>THIS MATERIAL EVENT CONTAINS A "CATEGORY 2" LEVEL OF RADIOACTIVE MATERIAL</br>Category 2 sources, if not safely managed or securely protected, could cause permanent injury to a person who handled them, or were otherwise in contact with them, for a short time (minutes to hours). It could possibly be fatal to be close to this amount of unshielded radioactive material for a period of hours to days. These sources are typically used in practices such as industrial gamma radiography, high dose rate brachytherapy and medium dose rate brachytherapy.
