The following information was received froThe following information was received from the state of New York via facsimile:</br>A routine inspection by the NYS Department of Health resulted in a finding that a Thermo Fisher Scientific Model 885326 Ser. No. B7131 point level gauge calibrator source had not been used since 2011. Licensee efforts to locate the device following the inspection were not successful. The source was officially deemed missing July 30, 2015. The device contained a 2 microCurie Strontium 90 source. The licensee states that at some time between the original installation in 2001 and 2003, the board on which the Sr-90 source was installed, failed and was replaced by a new electronic test board that did not require a radioactive source. Thermo Fisher did the installation. Neither the facility nor Thermo Fisher have any documentation showing the disposition of the Sr-90 source.</br>NYS Event Report ID No.: NYDOH-15-09</br>THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL</br>Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
