The following was received from the State … The following was received from the State of Texas via email:</br>On March 31, 2015 the Agency (Texas Department of State Health Services) was notified by the radioactive materials reciprocity coordinator that it received a notice of an engineering company scheduled to complete work on a nuclear source stuck shutter. The licensee (Westlake Longview Corporation) was called without contact. A message was left on voicemail. The engineering company was contacted and stated that it was hired to complete work on a sealed source gauge for level measurements due to a stuck shutter. The item is a Ronan Model 4F6S, Cs-137, 40 mCi, Serial Number M7352. The investigation into this case is ongoing.
