At 1553 CST on November 30, 2013, during t … At 1553 CST on November 30, 2013, during the performance of the High Pressure Coolant Injection (HPCI) fast initiation test, HPCI did not produce the required flow rate as specified by Technical Specification Surveillance Requirement 3.5.1.6. The HPCI subsystem is designed to ensure adequate core cooling when operated with other available Emergency Core Cooling Systems (ECCS), and during this event, all other ECCS systems remained operable. The cause of this event is currently under investigation.</br>This event is being reported in accordance with 10 CFR 50.72(b)(3)(v)(D) as a condition that could have prevented the fulfillment of the safety function of structures or systems needed to mitigate the consequences of an accident.</br>The NRC Resident Inspector has been notified.</br>The flow rate achieved during the test was 3000 gpm and the required minimum flow rate is 5000 gpm.he required minimum flow rate is 5000 gpm. +
