Via relay by the N.C. Division of Emergenc … Via relay by the N.C. Division of Emergency Management Operations Center, N.C. Radiation Protection Section was notified on 14 Jan 2009 by the Corporate RSO for MISTRAS Holding Group, doing business as Conam Inspection and Engineering Services, Inc/ Quality Services Laboratories, Inc. of a possible extremity over exposure. The possible over exposure occurred during a routine radiography source transfer between a QSA 660 camera containing 22 Curies of Iridium 192 (the old source) and an Ir-50 source exchanger containing 100 Curies of Iridium 192 (the new source). Difficulties in completing the exchange resulted in manual attempts to complete the transfer. The licensee is reporting at the end of the event all radioactive material was confirmed as being appropriately stored and secured. Dosimetry for the licensee staff involved in the event were overnighted to the dosimetry vendor for evaluation, results are pending. Details of the event and status confirmation are to be determined during an onsite investigation by the N.C. Radiation Protection Section on 15 Jan 2009.</br>NC event report #: NC-09-04n 15 Jan 2009.
