The following information was provided by … The following information was provided by the Louisiana Department of Environmental Quality (LDEQ) via email:</br>On March 3, 2026, the site work coordinator at the ExxonMobil refinery in Baton Rouge reported that a radiography camera had been found unattended. The camera was discovered by an employee of IRIS Inspection Company. </br>The licensee crew consisted of two state-carded radiographers and one instructor radiographer. The crew entered the unit, set up the barricade, and completed one exposure. After completing the exposure, they took down the barricade but inadvertently left the camera behind in the unit.</br>The camera remained unattended for approximately one hour and was returned to Acuren at approximately 0945 CST.</br>Camera: QSA Delta 880, S/N D3706</br>Source: 27.6 curie Ir-192, S/N 24192P</br>LA Event Report ID: LA 20260005</br>THIS MATERIAL EVENT CONTAINS A 'Category 2' LEVEL OF RADIOACTIVE MATERIAL</br>Category 2 sources, if not safely managed or securely protected, could cause permanent injury to a person who handled them, or were otherwise in contact with them, for a short time (minutes to hours). It could possibly be fatal to be close to this amount of unshielded radioactive material for a period of hours to days. These sources are typically used in practices such as industrial gamma radiography, high dose rate brachytherapy and medium dose rate brachytherapy.
