The catheter tubing became clogged during the administration. A new catheter was attached, and the procedure was completed. The staff was unsure on how to check the residual activity in the apparatus and contacted the manufacturer to help guide them in the process. When the residual activity in the initial and replacement catheters was measured, it was discovered that only approximately 46.2 percent of the prescribed dosage was administered (7.2 mCi of intended 15.57 mCi). The medical staff then reported the medical event to the radiation safety officer on February 10, 2025, who then reported it to NJDEP. The licensee will follow-up with a full written report.</br>A Medical Event may indicate potential problems in a medical facility's use of radioactive materials.
