The following information was provided by The following information was provided by the Colorado Department of Public Health and Environment (CDPHE) via email:</br>On Friday, January 30, 2026, at approximately 1525 MST, the radiation safety officer (RSO) for Memorial Hospital, University of Colorado Health, reported a medical event to CDPHE. The facility reported that at approximately 1430 MST a patient undergoing prostate radiation therapy treatment using Lu-177 (Pluvicto) had a medical event. The patient was on the second of six prescribed injections for this treatment protocol when the event occurred. The RSO stated that the intravenous injection (IV) line that was being used to administer the dose "blew" or otherwise became disconnected from other parts of the IV line. As a result, only 120 mCi (60 percent) of the prescribed 200 mCi was administered to the patient. Contamination of the surrounding area occurred, and access control protocols were implemented, along with restricting access to the dedicated injection room. Per the RSO, notification of the authorized user physician and referring physician is in process.
