The following report was received from the … The following report was received from the Ohio Department of Health via email:</br>On March 3, 2017, The Urology Center, LLC, Ohio Radioactive Material licensee no. 02200310002, reported an incident pursuant to which the total dose delivered differed from the prescribed dose by more than 20 (percent). The radiant source was brachytherapy seeds surgically inserted to the prostate. The incident occurred on March 2, 2017 and was confirmed on March 3, 2017 by a review of the post procedure CT (computed tomography) scan. The prescribed dose was 110 cGy using I-125 (model AgX100) seeds. The post CT plan shows that the actual dose delivered to the prostate was 27.6 cGy. The physicians do not believe that there are side effects to the rectum, urethra or neurovascular bundle as a result. </br>The patient has been informed.</br>OH NMED Item Number: OH170001</br>A Medical Event may indicate potential problems in a medical facility's use of radioactive materials.
