The following information was received via … The following information was received via facsimile:</br>The licensee discovered this event on October 15, 2016 and notified the Department (PA Department of Radiation Protection) on October 15, 2016 via voice message after business hours. It is reportable within 24 hours per 10 CFR 30.50(b)(2).</br>During normal daily operations, a density gauge was found to have a shutter that was nonresponsive and stuck in the open position. No overexposures have occurred and the gauge has been taken out of service. A service provider has already made the necessary repairs as of today, October 17, 2016. The gauge is an Ohmart/Vega Model SHLG. The serial number of the device is unknown at this time. The device originally contained 1200 millicuries of Cs-137.</br>The cause of the event is unknown at this time.</br>A reactive inspection is planned by the Department.
