The inspectors reviewed a self-revealing n … The inspectors reviewed a self-revealing noncited violation of 10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, for failure to identify a degraded equipment condition in December 2006. As a result, the emergency diesel generator system experienced a failure on October 22, 2009, which caused the plant to make a notice of unusual event emergency declaration. Licensee personnel missed an opportunity to identify the condition because they did not thoroughly evaluate a surveillance failure and post-mortem testing data available in December 2006.</br></br>The finding is more than minor because it was associated with the equipment performance attribute of the Mitigating Systems Cornerstone, and it affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Inspection Manual Chapter 0609, the issue is determined to have very low safety significance because the finding is not a design or qualification issue confirmed not to result in a loss of operability or functionality; did not represent an actual loss of safety function of the system or train; did not result in the loss of one or more trains of nontechnical specification equipment; and did not screen as potentially risk significant due to a seismic, flooding, or severe weather initiating event.
