The inspectors identified a non-cited viol … The inspectors identified a non-cited violation of 10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, for failing to correct a condition adverse to quality for lubricating the high pressure core spray diesel generator bearings. The station documented the finding in Condition Report CR-RBS-2012-02666. This performance deficiency was more than minor and was a finding because, if left uncorrected, inadequate lubrication work instructions could cause bearing failure due to inadequate lubrication or generator winding failure due to grease intrusion into the electrical windings in the generator. The significance of this finding was evaluated using a Phase 1 significance determination process screening and was determined to be of very low safety significance (Green) because it was not a design or qualification deficiency; did not represent a loss of system safety function; and did not screen as potentially risk significant due to seismic, flooding, or severe weather initiating events. The apparent reason the initial condition report was closed without correcting the work instruction to lubricate the high pressure core spray diesel generator bearings was that personnel who prepared and approved the operability evaluation were focused on proving operability not correcting a condition adverse to quality. Their focus was specific to the components ability to perform its function and not on completely identifying the issue in the corrective action program.
