On September 21, 2010, during a planned we … On September 21, 2010, during a planned weekly run of the Unit 2 HPCI auxiliary oil pump, a minor oil leak was discovered on the in-service lubricating oil filter and HPCI was declared inoperable (EN 46268) while the standby filter was placed in-service. A subsequent investigation determined that an undersized o-ring was installed in the filter which led to the degraded condition. This error is a violation of 10 CFR 50, Appendix B, Criterion VIII, Identification and Control of Material, Parts, and Components, for failure to prevent an incorrect component from being installed. The finding is more than minor because it affected the equipment performance attribute of the Mitigating Systems cornerstone and its objective of ensuring the availability and reliability of systems that respond to initiating events. It was also similar to example 5.c. in NRC IMC 0612 Appendix E, Examples of Minor Issues, which states, in part, that the failure to establish controls to prevent the wrong part from being installed in a system is more than minor if the wrong part is installed and the system is returned to service. The finding was evaluated for significance using IMC 0609, Attachment 4, Phase 1 - Initial Screening and Characterization of Findings. Since the finding did not result in a loss of safety function or the loss of a train for greater than its TS allowed outage time, and was not potentially risk significant due to external event initiators, the finding was determined to be of very low safety significance (Green).
