The licensee failed to establish the alarmThe licensee failed to establish the alarm setpoints for the pressurizer relief valve tailpipe temperature in accordance with their design basis. Title 10 of the Code of Federal Regulations, Part 50, Appendix B, Criterion III, Design Control, states, in part, that measures shall be established to assure that applicable regulatory requirements and the design basis, as defined in 50.2 and as specified in the license application, for those structures, systems, and components to which this appendix applies are correctly translated into specifications, drawings, procedures, and instructions. Contrary to the above, prior to November 7, 2011, the licensee failed to assure that the design basis for the pressurizer relief valve tailpipe alarm setpoints were correctly translated to specifications and procedures. Specifically, the pressurizer relief valve tailpipe setpoint was set at 280 degrees Fahrenheit in accordance with the setpoint study documented prior to initial startup. The licensee failed to adjust the setpoint to the design basis setting of the actual system temperature plus 25 degrees Fahrenheit as specified. This finding was assessed using Manual Chapter 0609, Appendix A, "The Significance Determination Process (SDP) for Findings at-Power," and was determined to be of very low safety significance (Green) because it did not result in the loss of a system or function.
