The team identified a non-cited violation … The team identified a non-cited violation of License Condition 2.C.9, "Fire Protection," for the failure to adequately correct a previous violation. Specifically, the licensee failed to provide a bounding calculation for the amount of time available for operators to establish component cooling water during an alternative shutdown. The licensee developed this calculation in response to Non-cited Violation 2012007-02. The licensee entered this issue into their corrective action program as Condition Report CR-WF3-2015-0859 and implemented a fire impairment as a compensatory measure. The failure to provide a bounding calculation for the amount of time available for operators to establish component cooling water during an alternative shutdown was a performance deficiency. The performance deficiency was more than minor because it was associated with the protection against external events (fire) attribute of the Mitigating Systems cornerstone and it adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. A senior reactor analyst performed a Phase 3 evaluation to determine the risk significance of this finding since it involved a postulated control room fire that led to control room evacuation and determined this violation was of very low safety significance. This finding had a cross-cutting aspect associated with resolution within the problem identification and resolution area since the licensee failed to take effective corrective actions to address issues in a timely manner commensurate with their safety significance.
