The inspectors reviewed a self-revealing G … The inspectors reviewed a self-revealing Green finding and an associated non-cited violation for the failure to follow Procedure OP-1052.007, Secondary Chemistry Monitoring, Revision 040. Specifically, the licensee failed to inject corrosion inhibiting chemicals into Unit 2 service water during refueling outages, which resulted in increased corrosion of the service water system. This issue was entered into the corrective action program as Condition Report CR-ANO-2-2016-02879. The failure to inject corrosion inhibitors into Unit 2 service water during refueling outages resulted in increased corrosion of the service water system is a performance deficiency. The performance deficiency is more than minor because it adversely affected the human performance attribute of the mitigating system cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences (i.e., core damage). Specifically, the performance deficiency adversely affected the structural strength of service water system boundaries. Using NRC Inspection Manual Chapter 0609 Appendix A, The Significance Determination Process (SDP) for Findings At-Power, Dated June 19, 2012, the inspectors screened the finding as having very low safety significance because it is a deficiency affecting the design or qualification of a mitigating SSC, but the SSC maintained its operability. The inspectors determined that this finding had a cross-cutting aspect in the human performance area of Avoid Complacency, because the licensee failed to recognize the potential consequences of isolating chemical injection to the service water during outages, which contributed to degradation.outages, which contributed to degradation. +
