A self-revealing non-cited violation of 10 … A self-revealing non-cited violation of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures and Drawings, was identified for the failure of work control and maintenance personnel to follow the procedure requirements for work on a reactor coolant system pressure retaining component. Specifically, work control and maintenance personnel did not use work documents and procedures to reassemble the vent valve for the control element drive mechanism associated with control element Assembly 22, which resulted in a reactor coolant system leak during the fill and vent process. This finding was entered into the licensees corrective action program as Nuclear Notification 200323460. The finding is greater than minor because it is associated with the reactor coolant system equipment and barrier performance attribute of the Barrier Integrity Cornerstone and affects the associated cornerstone objective to provide reasonable assurance that physical design barriers protect the public from radionuclide releases caused by accidents or events. Using Manual Chapter 0609, Significance Determination Process, Appendix G, Shutdown Operations Significance Determination Process, Checklist 4, the finding was of very low safety significance because it did not increase the likelihood of a loss of reactor coolant system inventory by more than two feet when not in a mid loop operation.
