The following violation of very low safetyThe following violation of very low safety significance (Green) was identified by PSEG and is a violation of NRC requirements which meets the criteria of the NRC Enforcement Policy for being dispositioned as a NCV. TS 3.3.2.1, Engineered Safety Feature Actuation System Instrumentation, requires the channels and interlocks shown in Table 3.3-3 to be operable with their trip set-points set consistent with the values shown in the trip set-point column of Table 3.3-4. Specifically, automatic actuation logic for the main steam line isolation and auxiliary feedwater is required to have two operable channels, but only had one operable channel following the inadvertent train A safety injection signal on April 30, 2012. Contrary to TS 3.3.2.1, Table 3.3-3, Action Statement 20, Mode 4 was not entered in the required time by 2:28 AM on May 1, 2012. PSEG determined that action statement 20 was applicable at 8:02 AM on May 1, subsequently entered the action statement, commenced cool-down, and entered Mode 4 at 5:39 PM. PSEG entered this issue into the CAP as notification 20557859. The inspectors determined that the finding was of very low safety significance (Green) in accordance with NRC IMC 0609, Attachment 4, Initial Screening and Characterization of Findings, Mitigation Systems, because the finding was not a design or qualification deficiency, did not result in an actual loss of safety function, and was not potentially risk significant for external events.ally risk significant for external events.  +
