A finding of very low significance was ideA finding of very low significance was identified by the inspectors for the licensees failure to implement Procedure EN-HU-102, Human Performance Tools, which established standards and expectations for the use of specific human performance tools with the goal to improve personnel and plant performance through human error reduction. The inspectors identified that Procedure EN-HU-102 was not implemented for the work performed on September 25, 2011, to install a temporary modification and to address a non-conforming condition associated with Panel D11-2. Implementation of the procedure for Panel D11-2 scheduled work required the use of Procedure EN-OP-116, Infrequently Performed Tests or Evolutions, and performance of an infrequently performed tests and evolution pre-job brief, which the inspectors determined was not performed for the work on September 25, 2011. No violation of NRC requirements occurred. The licensee documented this condition in its corrective action program as CR-PLP-2011-04822 and CR-PLP-2011-04981. At the end of this inspection, the licensee continued to perform a root cause evaluation to determine the causes of the event and develop corrective actions.
