| ENS 52422 | 9 December 2016 17:21:00 | While preparing to perform his first exposure of the day, the radiographer determined that the source became stuck near the camera and guide tube connection. He notified the client, verified his boundaries, and contacted the Radiation Safety Officer (RSO). After discussion with the radiographer, the RSO believed the source was lodged due to freezing temperatures and a small amount of ice within the guide tube. The radiographer was able to obtain a heater and, after about an hour of warming the guide tube, was able to retrieve the source into the camera normally. No overexposures occurred due to this event. The camera is a SPEC-150 that contains 47 Ci of Ir-192. |
| ENS 51963 | 27 May 2016 16:56:00 | Radiographers were performing an exposure when the guide tube disconnect failed and the guide tube disconnected from the camera. This prevents the source from being fully retracted into the camera. The division manager, trained in source retrieval, was able to get the source into the camera and into its shielded position. No overexposures occurred. The quick disconnect was replaced and tested and the camera was returned to service. The camera was a SPEC-150 using a 62 Ci Ir-192 source. |
| ENS 46192 | 20 August 2010 18:24:00 | JANX crew was working on a piping fabrication. A piece of 2 inch pipe fell while making an exposure. It is believed that the falling pipe struck the guide tube. The radiographer then attempted to retrieve the source. The source partially retracted and then became stuck. The area was then cordoned off. The area is being well controlled. JANX's customer is Dominion Gas Transmission. There are no residential units close to this area. Source Production & Equipment Co. Inc. (SPEC) has been contacted. SPEC is dispatching personnel to effect source retrieval. No overexposures resulted from this event. The radiography camera is a SPEC model 150 containing 90 curies IR-192. GPS coordinates of event location: 39.10699 N 80.49879 W Notified R1DO (White) and FSME EO (Delligatti). |
