The following information was excerpted from a facsimile from AMETEK Solidstate Controls: Ohmite 5 watt, 470 ohm wire wound resistor with component Date Code 1201 is mounted on a printed circuit board installed in power supplies manufactured by Ametek Solidstate Controls. Component Description: Ohmite 5 watt, 470 ohm wire wound resistor with Date Code 1201 Problem You Could See: Internal open - (100 hour burn-in performed) Probable Cause: Insufficient spot weld on the wire wound resistor Effect on System Performance: Resistor opens Action Required: Notify NRC and affected utilities. Prepare replacements. Ametek Solidstate Controls Corrective Action Taken: Notified affected utilities of the potential defect and advised that the power supplies be returned to our factory. Further corrective action will be to install new boards with unaffected resistors, retest, perform 100 hour burn in and return. ENS 4774114 March 2012 15:38:00The following information was provided by facsimile: COMPONENT DESCRIPTION: Arvan electrical terminal blocks, part number MS27212-x-xx, Revision P, sizes -3-xx and -5-xx. any length; Ametek Solidstate Controls part number 03-120608-00. Only lots purchased from 2010 to present are of concern. PROBLEM YOU COULD SEE: A mottled or marbled appearance, vertical cracks and/or a slim void/flow line on a cut section of the terminal block, CAUSE: The manufacturer's investigation revealed that an improper application of acetone agent, which ensures proper bonding/blending of materials, was not properly monitored and resulted in intermittent inadequate blending. Not every lot or complete batch run is affected. EFFECT ON SYSTEM PERFORMANCE: Cracks and/or voids in the material could be exploited should a seismic event occur. ACTION REQUIRED: We have no known failures of these terminal block products in our systems. We believe the failure rate has been extremely low and the risk is minimal. Each operating facility will need to evaluate the potential risk to their operation. ENS 482857 September 2012 12:45:00PRODUCT: 10 KVA inverter Model 86VC0100-15, 3 phase 480 VAC output. Limited to systems supplied to Exelon Byron and Braidwood units. Capacitors C10 through C15 part number 03-040060-00. COMPONENT DESCRIPTION: Suppression capacitors, Ametek part number 03-040060-00, used in positions C10 through C15 on Model 86-VC0100-15 only. PROBLEM YOU COULD SEE: Capacitor could arc internally and open internally and externally. CAUSE: the manufacturer's investigation revealed that due to improper selection of capacitor used may fail due to a corona effect internally. EFFECT ON SYSTEM PERFORMANCE: The capacitor may short internally and open circuit by forcing lead separation from the device. ACTION REQUIRED: Replace the capacitor(s) with Ametek part 03-040041-20. The replacement is rated at 370 VAC and used in a 277 VAC circuit. AMETEK SOLIDSTATE CONTROLS CORRECTIVE ACTION: The defect is limited to the units supplied to Exelon Byron and Braidwood plants under Ametek joc C103552. Both the Byron and Braidwood plants are already informed of this condition.
ENS 4730128 September 2011 17:23:00The following was received via fax: Component Description: International Rectifier (IR) and Vishay 150 amp clamp diodes with either forward or reverse bias These are Ametek Solid-state Controls part numbers 07-600150-00 and 07-600151-00. Diodes can be either installed in Ametek Solid-state Controls UPS equipment or provided as a spare part. The diode failures due to this suspected defect have occurred generally at around the third or fourth year of operating life of the device. There are no warning signs that a failure is imminent, or detection method for predicting an approaching failure. The investigation has revealed that the diode failures were due to voltage transients or punch through. A failure analysis by Southwest Research concluded that the device having an 'alloy junction' can deteriorate after three to four years of operation resulting in a 'punch through' condition within the device causing the diode to short. The actual cause has not been determined; however it is suspected that the alloy junction type device may have sensitivity to age or voltages causing the device to more rapidly degrade. The failures described above, could result in loss of output voltage and transfer of the static switch to the bypass source which could result in a potential damage to the load. We have only two known failures in systems at this voltage level. We feel the failure rate has been extremely low and the risk is minimal. Each operating facility will need to evaluate the potential risk to their operation. The licensee did not indicate which NRC licensees, if any, are affected by this notice.
ENS 4480526 January 2009 11:32:00The following information was received from AMETEK via facsimile: COMPONENT DESCRIPTION: Electro-mechanical 14 pin relays, with AC voltages of 120 and 240, and DC voltages of 12, 24, 48 and 110. The Tyco/(Potter & Brumfield) relays were removed from the Approved Supplier Listing in 2006. The relays can be installed in Ametek Solidstate Controls equipment, or provided as a spare part. PROBLEM YOU COULD SEE: Nuisance alarming or erratic operation of the equipment. CAUSE: The problem appears to be an age related degradation of unknown cause. A failure analysis performed by First Energy Laboratory Services identified a potential cause as a crinkled appearance on the surface of the moving contact pads resulting in uneven contact with the opposing stationary contact pad. Over time, a layer of oxidation forms on the pads inhibiting electrical contact and increasing resistance. EFFECT ON SYSTEM PERFORMANCE: Potential unstable output voltage caused by intermittent high relay resistance. The relays are generally used for alarm functions but can be used in control circuitry as well. ACTION REQUIRED: Replace all Tyco/Potter & Brumfield relays listed below remaining in service with the approved Ametek Solidstate Controls equivalent. These types of AMETEK equipment are installed at various reactor licensees. There was no information on which licensees are affected. The manufacturer received notification of the faulty relays from Beaver Valley Power Station. The problem was revealed through a malfunction of a battery charger on 12/11/2008. The manufacturer does not believe that there is an immediate safety concern.
ENS 425514 May 2006 13:23:00The following information was provided via email: COMPONENT DESCRIPTION: 13uF X 1000V AC capacitors, SCI P/N 07-020139-00, Aerovox P/N H94S1013A0BA, manufactured between January 2004 and December 2004. Capacitors can be either installed in Ametek Solidstate Controls UPS equipment or provided as a spare part. The components are schematically identified as C805, C815, or C705. PROBLEM YOU COULD SEE: The capacitor failures due to this defect will occur early in the operating life of the device. There are no warning signs that a failure is imminent, or detection method for predicting an approaching failure. CAUSE: The investigation has revealed that the capacitor failures (date codes 0408 and 0436) were due to manufacturing defects. A failure analysis by Aerovox revealed that a foil tab connecting the capacitor winding to the internal terminal became pinched between the lid and the can during the seaming operation. Within the first year of operation, the insulation covering the connecting tab was cut through. This resulted in terminal to case short. EFFECT ON SYSTEM PERFORMANCE: The failures described above, could result in loss of output voltage and transfer of the static switch to the bypass source. The resulting failure could result in a potential damage to the load. ACTION REQUIRED: The observed failure rate of this component is very low at 0.06% (2 out of 3336 units manufactured between 1/1/04 and12/31/04.) Aerovox has implemented corrective actions to preclude further failures. Based upon the Aerovox evaluation, Ametek Solidstate Controls believes if a defect was present in a capacitor, the likelihood of a failure should occur early in the operating life. We feel the failure rate has been extremely low and the risk is minimal. Each operating facility will need to evaluate the potential risk to their operation.
