ENS 5586227 April 2022 13:19:00The following is a summary of information received from the Colorado Department of Public Health and Environment by e-mail: The licensee reported a damaged Troxler 3430 portable moisture density gauge (SN:24886) containing Am-241/Be (SN:4721079) and Cs-137 (SN:757025) sources. The gauge fell out of the back of a truck onto an interstate highway while in transit. Pieces of the gauge are on the interstate as a result of vehicles running over the gauge and the licensee is attempting to retrieve them. The licensee intends to contact the police to help cordon off the affected area to support retrieval of the remaining pieces and conduct surveys. A search is ongoing and some of the pieces of the gauge have not been accounted for. CO Incident Number: CO220013 THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. ENS 5395625 March 2019 16:37:00The following was received via e-mail: The department (Colorado Department of Public Health and Environment) received an e-mail notification on March 7th, 2019 reporting two leaking Electron Capture Devices (ECD's). The (ECD's) are associated with Agilent model G239765505 with serial numbers: U1186 and U6642. The semi-annual leak test results show 35300 picocurie/sample for serial number U1186 and 17100 picocurie/sample for serial number U6642. The wipe test results were received by Test America on March 4, 2019. Test America reported that the ECD's were removed from use and the instruments on March 4, 2019 and sent to Detector Service Center, LLC for cleaning and repair. Test America performed surveys for the affected machines and cleaned with a radiacwash solution. A wipe test was done on the instruments and counted by Gas Flow Proportional Counter low background instrument which indicated no removable contamination present. A direct survey with a Ludlum Model 3 Giger Muller detector indicated no fixed contamination. The completion date was reported to be March 5, 2019. Test American has removed the leaking ECD's and will continue 6-month wipe tests as required. Colorado Event Report ID No.: CO190004
ENS 5390128 February 2019 17:15:00The following was received via email from the state of Colorado: At approximately 1438 MST on February 28, 2019 the RSO (Radiation Safety Officer) for CO 152-01, St. Anthony Hospital - Centura Health, notified the Department (Colorado Department of Public Health and Environment) of a Y-90 SIR-Sphere misadministration. The procedure occurred at approximately 1300 MST and it was determined that 65 percent of the prescribed dose was delivered to the patient. The RSO noted that during the procedure there were issues with the delivering catheter. During the procedure, the catheter was replaced. A department investigation will occur in the near future. A written report per section 7.21.4 is expected within 15 days. ENS 5334619 April 2018 18:16:00The following was received from the State of Colorado via email: The Department (Colorado Department of Public Health and Environment) was notified via phone on 4/19/18 at approximately 1552 MDT. A G Wassenaar's RSO notified the Department that a portable gauge had been run over by a vehicle and the vehicle did not stop. At the time of the phone call, the gauge was broken into multiple pieces and the RSO was about to go out on-site to assess the damage. State inspectors are responding immediately as well. ENS 5233028 October 2016 17:42:00The Department (Colorado Department of Public Health and Environment) was notified via phone on 10/28/16 at 1309 MDT by the Radiation Safety Officer (RSO) of Colorado State University (license # CO 002-19) that an irradiator room with a panoramic irradiator (J.L. Shepherd and Associates Model 81-14A irradiator, Cs-137 sealed source, 6000 Ci at 11/01/1976) has been disabled. The reason to disable the irradiator is because the sample drawer is stuck. The licensee reports the source is (apparently) fully in the shielded position because: the incident occurred before operating the irradiator, Primalert monitors are not alarming, and surveys do not suggest the source is exposed. At approximately 1217 MDT, the licensee was to perform a monthly check of the safety system. It was a normal entry into the irradiator room. The irradiator source was not exposed. The licensee checked the operation of the sample drawer and it was found to be stuck, and cannot be opened. The sample drawer is to be used to place a sample into the housing closer to the source for irradiation. The irradiator source was not moved at any point. The RSO was then notified. The keys to the irradiator room are currently secured in a locked box where no authorized operators to the irradiator can access. An e-mail was sent out to authorized operators that the irradiator is out of service. The RSO will be contacting J.L. Shepard & Associates on Monday 10/31/16 for service options. The Department will be notified on Monday 10/31/16 after J.L. Shepard & Associates communication. Colorado Event Report ID No.: CO16-I16-27
ENS 5138911 September 2015 16:07:00The following report was received from the State of Colorado via email: Event description: The Department (Colorado Department of Public Health and Environment) received a phone report from (licensee) on 09/11/15, at 0906 (MST). It was reported that a source disconnect occurred on 09/10/15, at about 1745. At that time, the licensee RSO (Radiation Safety Officer), went out and retrieved the source according to procedures for source retrieval. The source was successfully retrieved, and no over exposure was reported. The Department (Colorado Department of Public Health and Environment) is preparing for a visit to investigate the event and is expecting a full report from the licensee within 30 days. No reported over exposure from this event. No additional information on the Radiography Camera or source strength is available at this time. Event Report ID No.: CO15-I15-26
