ENS 573086 September 2024 13:06:00The following information was provided by the Florida Bureau of Radiation Control (BRC) via email: BRC received notification from the Advent Health Altamonte radiation safety officer of a possible medical event. A patient received a Y-90 TheraSphere treatment, intended for the liver, which migrated to the stomach. The prescribed dose was 250 Gy, and the dose assessment is still being conducted. The facility states a majority of the prescribed dose was received by the stomach. Migration was identified by the patient's physician, and the patient was notified this morning. Florida incident number: FL-24-082 A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
ENS 5729026 August 2024 12:46:00The following was reported by the Florida Department of Health, Bureau of Radiation Control (BRC), via email: The BRC received notification from Universal Engineering Sciences of a Troxler gauge being hit by a passing car. The Troxler gauge was being standardized by a technician when a passing car made contact with the device. The source rod was retracted at the time and has not been exposed. The only identified damage was a crack in the plastic near the LCD screen. The following information was provided by the Florida Bureau of Radiation Control (BRC) via email and phone: On May 30, 2024, at 1710 (EDT), BRC received a notification from Cape Coral Fire (department) concerning a deceased individuals body exploding while being cremated. The BRC received this notification because the deceased individual was known to have been receiving pancreatic cancer treatment. A notification was also received from the State watch office. The son of the deceased stated that he did not believe that the deceased had received treatment within the last two months, but he was unsure. Crematory staff stated that a large flame exited the bottom of the oven and they began to feel sick. A crematory employee stated that the body completed the cremation process in half the time expected. The Cape Coral Fire (department) responded. Background readings were observed at 3 micro R/hr and readings within the incident room registered at 10 micro R/hr. An inspector has been assigned by regional manager for immediate response. The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance: The contaminated areas have been isolated and locked until the inspector arrives. ENS 570001 March 2024 12:48:00The following summary of information was provided by the Florida Bureau of Radiation Control (the Bureau) via email: On March 1, 2024, at 1121 EST, the Bureau received a call from Universal Engineering Scientists to report that a Troxler gauge (Model: 3430P, Serial: 86000, 8 mCi Cs-137, 40 mCi Am-241:Be) was run over on a work site. The Cs-137 source rod was extended 12 inches into the ground. The licensee radiation safety officer (RSO) responded and determined that the source rod could not to be retracted. The gauge was placed in a container and shielded for transport to a storage facility in Port St. Lucie where it will be held for evaluation. The Bureau inspector has been notified and will respond. ENS 5675422 September 2023 16:16:00The following was reported by the Florida Bureau of Radiation Control (BRC) via email: At 1537 EDT on 9/22/23, BRC received a call from Cleveland Clinic stating an I-125 source was lost after removal from a patient. The patient was suspected to have breast cancer, and two I-125 sources were implanted in the breast tissue. The patient returned home 9/18/23 with the implanted seeds, and returned to the clinic 9/20/23 to receive a Tc-99m injection prior to tissue removal. The licensee states they confirmed both seeds were no longer in the patient after surgery, however, when the samples were taken by pathology, only one seed was located in the removed tissue. Cleveland Clinic has completed surveys of the operating room and pathology. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. ENS 5662213 July 2023 10:08:00The following information was provided by the Florida Bureau of Radiation Control (BRC) via email: BRC Orlando was notified on 7/13/23 that a device leased by Tesla Inc., Tampa from NRD, LLC is lost. The device is an ion gun initially containing 10 mCi of Po-210. The licensee, Tesla Inc., did not turn up in a search of the general licensee Laserfiche (content management platform). BRC has reached out to Tesla Inc. to get further details and have not yet received a response. Tesla Inc. is in the process of applying for a license (1559-1). A BRC Inspector will respond. ENS 565555 June 2023 11:42:00The following was received by email from the Florida Department of Health, Bureau of Radiation Control (BRC): BRC received a call at 1045 EDT from the radiation safety officer of ECS Florida LLC. The caller stated that an Instrotek 3500 (#4012, AmBe K893/19, Cs137 BG1008) moisture density gauge was stolen from the back of a pickup truck on its way to a job site. The vehicle made several stops on the way and they do not know at which location it was stolen.
