The following information was received from the Oklahoma Department of Environmental Quality (DEQ) via email: On 5/17/2022, the state of Arkansas notified Oklahoma DEQ's Radiation Management Section that a DOT ((Department of Transportation)) Special Permit 10656 had been issued for a trailer that originated in Sallisaw, Oklahoma. The load was surveyed at the scrap metal facility in Arkansas and sent back to Oklahoma. It arrived back to Sallisaw today and was surveyed by the owner of the salvage yard. The owner of the salvage yard found a Thermo Measure Tech Model 5201 fixed gauge containing 50 mCi of Cs-137, serial number R4473. The source housing appears to be intact. When the gauge was removed from the trailer no additional radiation was measured on the trailer. The facility owner was instructed to isolate and secure the gauge. The owner stated that he would place it in a drum and lock it in a concrete shed. DOT Approval Number: AR-OK-22-007

The following information was provided by the DEQ via email: The manufacturer of the gauge provided (DEQ) with the name of the company that purchased the gauge; they are still a valid company based out of New Jersey. (DEQ) has contacted them and are waiting for a response. (DEQ) searched NMED ((Nuclear Material Events Database)) but did not find any reported loss or theft of this gauge. The following information was provided by the DEQ via email: Using the manufacturer-provided information, we found that the gauge had been transferred to Illinois. The Illinois Agreement State program found that it had been sold to Mid America Dredging in Good Hope, IL. The state of Illinois is following up with their registrant and presumably will report on events. Oklahoma DEQ will assist as needed. ENS 5513512 March 2021 11:29:00

The following was reported by the state of Oklahoma via email: Globe X-ray Services, Inc., a radiography company, reported via phone this morning that a technician's dosimeter received a 5.3 rem dose. The dose was reported on the January dosimetry report and the licensee learned about it yesterday. The licensee is currently working with Landauer (badge supplier) to have the badge processed further, and is investigating the exposure. No additional information is known at this time, we will provide updates as additional information becomes available. Oklahoma event Number: Not Assigned

The following update was received form the state of Oklahoma via email: The badge reading was: 5132 mrem DDE, 5192 mrem LDE and 5192 mrem SDE. The RSO stated that they believe the Landauer dosimeter received the dose on January 22, 2021. The RSO interviewed the radiographer and it is believed that the Landauer dosimeter was in a jacket pocket in the shooting area during exposures. The alarming ratemeter and pocket dosimeter readings were not elevated for January 22, 2021. The licensee has contacted Landauer and are awaiting imaging results to determine if the exposure was static. Notified R4DO (Kellar), and NMSS Events Notification via email.

The following update was received form the state of Oklahoma via email: The licensee's investigation determined that the overexposure was to the badge, not to the individual. The licensee reported that the radiographer removed his jacket containing his dosimeter and laid the jacket in the area near where radiographs were being taken. He made approximately 12 exposures with a 56 curie Ir-192 source with the jacket in the area. The radiographer's direct reading dosimeter was not off scale and his alarming ratemeter did not alarm. The second radiographer on site also reported that the individual was using his survey meter. Results from Landauer were inconclusive regarding the exposure being static or dynamic. Based on the licensee's findings, they have requested that Landauer adjust the recorded dose to the individual as 114 (milliRem) for January 2021." ENS 5439920 November 2019 17:04:00The following was received from the State of Oklahoma via email: (The Oklahoma Department of Environmental Quality (ODEQ)) was notified by phone today that on November 6, Oklahoma licensee Blackwell Hospital Trust Authority (OK-32140-01) had administered a dose of Tc-99m Sestamibi to a patient that (the dose) was 24-hours old. The RSO reported that she had learned of the incident today and that the dose was administered to the intended patient. The activity of the dose originally is unknown at this time. The licensee will be investigating the event further to determine the details of what happened. (The ODEQ) will update this notice with more information as (the ODEQ) receives it." A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
ENS 5127630 July 2015 09:48:00The following report was received from the State of Oklahoma via e-mail: At about 0830 (CDT) American Piping Inspection, Inc., (license) OK-27438-02 (Tulsa, OK) reported to (the State of Oklahoma) that one of their radiography trucks was stolen and that there was a radiography camera on board. The truck has GPS and the company had tracked the truck to North Garnett Street in Tulsa, OK and the police were enroute. (At approximately 0930 CDT), "the radiography camera was recovered by the company (American Piping Inspection, Inc). The thieves were in the truck when the police arrived. The thieves departed the scene on foot and were not apprehended. There was no apparent damage to the truck or the camera. Device Model Number: Spec. G-60, containing 100 curies of IR-192. THIS MATERIAL EVENT CONTAINS A "CATEGORY 2" LEVEL OF RADIOACTIVE MATERIAL Category 2 sources, if not safely managed or securely protected, could cause permanent injury to a person who handled them, or were otherwise in contact with them, for a short time (minutes to hours). It could possibly be fatal to be close to this amount of unshielded radioactive material for a period of hours to days. These sources are typically used in practices such as industrial gamma radiography, high dose rate brachytherapy and medium dose rate brachytherapy.
