ENS 5575424 February 2022 10:11:00The following information was provided by the state of Louisiana via email: On February 24, 2022, at 0759 CST, the Louisiana Department of Environmental Quality received this event notification. The facility involved was Louisiana Integrated Polyethylene JV, LLC. On February 23, 2022, at 1800 CST, a Berthold device cable broke or disconnected in a vessel. The source cannot be retrieved back into a shielded condition. The gauge is a Berthold Model: SSC-200, source holder: LB 300 IRL, source S/N: 1405-11-16, 1000 mCi of Cs-137. Survey meter readings at the vessel are 400 mR/hr and 2 mR/hr at fifteen (15) feet. The facility has contacted the manufacturer to come assist in source retrieval within the pipe inside the vessel. ENS 556379 December 2021 10:27:00This information was provided by via email from LA Department of Environmental Quality (LDEQ): On December 8, 2021, LDEQ was notified by Halliburton Energy Services, Inc., Senior Radiation Safety Officer via email concerning well logging sources that were abandoned downhole. According to the RSO, the two sources were one 555 GBq (15 Ci) Eckert and Ziegler Model LWD4173, source s/n EZ16008 and one 74 GBq (2.0 Ci) QSA Global, Model CDC.CY13, source s/n 54903B. Halliburton was performing directional drilling for Whitney Oil & Gas LLC. The well was in inland waters. The date of occurrence was November 16, 2021. ENS 525356 February 2017 12:40:00The following report was received from the State of Louisiana via email: Event description: On February 6, 2017, LDEQ (Louisiana Department of Environmental Quality) was notified by Alpha-Omega, Inc., that the facility had received three 'EMPTY' Nucletron V2 containers from a Mexico site then transported to the United States, Alpha-Omega Services, Inc., Vinton, Louisiana, 70668. When inspected, two of the containers had Ir-192 sources in them, Model 105.002. Container 2785C6 had source D36F3728, 25 mCi of Ir-192, a QSA Global source. Container 2053C6 had source D36P3481, 351 mCi of Ir-192, an Alpha-Omega, Inc. source. Event Report ID No.: LA-170002.
