ENS 5038420 August 2014 18:21:00

On 8/20/2014, 0820 hrs (EDT), the Plant Shift Superintendent was notified of the following condition: During a maintenance evolution to change the machine drive unit (MDU) software, one of the four primary isolation valves comprising the machine isolation valve (MIV) set did not close as required. Specifically, the purge vacuum (PV) isolation valve did not close. All other isolation valves did close as required. This event is being reported as a loss or degradation of an IROFS (Item Relied on for Safety) that results in a failure to meet the performance requirements of Section 70.61 (and is reportable per 70.50(b)(2)). The licensee will notify the NRC Resident Inspector.

On 09/08/2014, the Plant Shift Superintendent, with concurrence of USEC-ACP RO (Regulatory Oversight) determined that this event can be retracted. Engineering Evaluation, EE-2101-0053 Rev. 0, establishes the basis for retracting Event Number 50384. The retraction is based upon an analysis showing that failure of the purge vacuum (PV) isolation valve in the machine isolation valve (MIV) IROFS does not create a condition that results in a failure to meet the performance requirements of 10 CFR 70.61. The licensee will notify the NRC Resident Inspector. Notified R2DO (Seymour) and NMSS EO (Habighorst).

ENS 4520816 July 2009 17:31:00

An ALERT was declared at 1715 due to a fire lasting greater than 15 minutes. The fire is in an abandoned cooling tower (bldg X-633D) on the Department of Energy portion of the site. The fire is not in proximity to any safety related equipment. A request for assistance was made to Scioto and Adams counties Fire Departments. At 1917, the licensee reported that the fire was out and the firemen were removing siding and looking for residual hot spots.

The fire is out and the ALERT has been terminated. Recovery operations are in progress and a fire watch will be stationed throughout the night. Notified R2DO (O'Donohue), R2 Deputy RA (McCree), NMSS (Weber), NMSS EO (Kotzalas), DHS (Kettles), FEMA (Biscoe), DOE (Parsons), HHS (Nunn), USDA (Timmons), and EPA (Threatt).

ENS 4498814 April 2009 13:40:00

On 04/14/09 at approximately 0918 hours, the Plant Shift Superintendent was notified of an oil spill that occurred on the plant site. A diesel generator sitting near a construction area was leaking fuel. Primary and secondary containment was initiated to contain the spill. At 1130 hours environmental response personnel determined that the quantity of oil spilled exceeded the Reportable Quantity (RQ). At 1144 hours, 04/14/2009, the Plant Shift Superintendent notified the National Response Center and the Ohio EPA that a Reportable Quantity (RQ) of oil (diesel fuel) was released to the environment. Ohio EPA assigned incident #: 0904-66-1084. National Response Center assigned incident #: 902636 ACD2-RG-044, Appendix N, section 1 requires an NRC event notification when other government agencies are notified. The amount of fuel oil spilled was 25 to 40 gallons. Licensee will be issuing a press release about this spill.

The licensee is issuing a press release and has revised the estimated spill to 300 gallons. Notified the R2DO (Widman), NMSS (Kokajko), and PAO (Hayden) via e-mail.

This event occurred at the American Centrifuge Plant (Docket 70-7004, license SNM-2011) and not the gaseous diffusion plant. Notified the R2DO (Widman) and NMSS (Kokajko) via e-mail.

ENS 4162322 April 2005 10:55:00On 04/21/05 at 2013 hours, Autoclave #2 in the X-343 Facility experienced a Steam Shutdown due to high condensate level alarms (A) and (B) actuating. The autoclave was in an applicable TSR mode (Mode II, Heating) at the time of the alarm actuations. The autoclave was placed in Mode VII (Shutdown) and declared INOPERABLE by the Plant Shift Superintendent (PSS). An investigation is underway to determine the cause of the actuations. No release of radioactive material occurred as a result of this incident. This event is being reported in accordance with UE2-RA RE1030, Appendix D. J. 2. Safety System Actuations. NRC, Region II has been notified of this event.
ENS 413919 February 2005 15:46:00The following information was obtained from the regulatee via facsimile: On 2/09/05 at approximately 1025 hours (EST) , a Hazardous Material spill occurred on plant site. Utility operations was transporting a small tank (app. 320 gallon capacity) of Sodium Hypochlorite with a fork truck. The tank tipped off of the forks, hit the ground, and landed on its side causing the cap to come off. Approx. 150 gallons of Sodium Hypochlorite spilled to the ground resulting in a hazardous material spill. At 1203 hours, the Plant Shift Superintendent notified the National Response Center and the Ohio EPA that a Reportable Quantity (RQ) of hazardous material (Sodium Hypochlorite)) was released to the ground. No material left the immediate area and clean-up of the spill is currently in process. UE2-RA-RE1030, Appendix D, section P requires a 4-hour NRC event (notification) when other government agencies are notified. There were no personnel injuries or plant equipment damaged as a result of the spill. The licensee will be notifying the NRC Resident Inspector.
