ENS 5708218 April 2024 05:00:00Agreement StateStuck ShutterThe following was received from the Texas Department of State Health Services (the Department) via email: On April 18, 2024, the Department was notified by the licensee that the shutter on a Vega model SH-F1 nuclear gauge failed to close. The gauge contains a 20 millicurie (original activity) cesium - 137 source. Open is the normal position for the gauge shutter. The licensee reported there is no risk of additional radiation exposure to members of the general public or radiation workers, due to this failure. The manufacturer has been contacted to repair the gauge shutter. Additional information will be provided as it is received in accordance with SA-300. ENS 5412620 June 2019 05:00:00Agreement StateAgreement State Report - Shutters Stuck Open on Three Fixed GaugesThe following report was received from the Texas Department of State Health Services via email: A licensee consultant reported, with authorization from the licensee RSO, that three fixed gauges; two Vega model SHF1 devices, serial no.'s 5873CN and 5801CN, containing 20 mCi Cs-137 and one Ronan SA1, SN 9500gg, containing 1000 mCi Cs-137 were found to have stuck shutters during routine inspection. The shutters are in the open position, which is their normal operating mode. The gauges remain in positon on tanks containing alkaline media and are still operating. A service company is being contacted to conduct repair/replacement. None are scheduled at this time. The gauges represent no hazard to the public due to their position. Texas Incident # 9689
ENS 4868319 January 2013 06:00:00Agreement StateAgreement State Report - Nuclear Gauge Shutter Stuck in Open Position

The following report was received from the State of Texas via email. On January 17, 2013, the licensee notified the Agency (Texas Department of State Health Services) that while performing preparations for maintenance the shutter on a Ronan model SH1 nuclear gauge was found stuck in the open position. The gauge contains 1,000 millicuries of cesium-137. The licensee stated that open is the normal operating position for the shutter and the gauge does not pose an exposure risk to any individual. The licensee stated that a service provider will remove the gauge and the manufacturer will be contacted to repair the gauge. Additional information will be provided as it is received in accordance with (Reporting Materials Events) SA-300. Texas incident # I-9033.

The following information was received by email: On January 22, 2013, the Agency was informed by the service provider that on January 14, 2013, they were able to get the shutter to close. The shutter is operating normally. The licensee has contacted the manufacturer to perform preventive maintenance on the gauge. Additional information will be provided as it is received in accordance with SA-300. Notified the R4DO (OKeefe) and FSME Events via email.
