| ENS 51667 | 18 January 2016 05:00:00 | Agreement State | Agreement State Report - Failed Shutter Assembly on Level Control Gauge | The following information was received from the Commonwealth of Pennsylvania via E-mail: The Department (Pennsylvania Department of Environmental Protection) was notified on January 19, 2016, that during routine activities on January 18, 2016, the weld on the collimator block, which is part of the shutter assembly to the gauge, failed. This event is reportable per 10 CFR 30.50(b)(2). The weld on the collimator block failed on a Thermo Measure Tech level control gauge. The shutter was closed at the time of the event and no workers received any dose. The licensee notified a service provider who responded and installed a new collimator assembly. Device Information: Manufacturer: Thermo Measure Tech Model: 5204 Device Serial Number: B497 Isotope: Cesium-137 Activity: 4 curies (The cause is attributed to) equipment failure. | ENS 51591 | 4 December 2015 05:00:00 | Agreement State | Agreement State - Two Gauge Shutters Failed to Close | The following report was provided by the Pennsylvania Department of Environmental Protection via facsimile: Two separate shutter issues occurred during gauge standardization procedures on December 4, 2015. The shutters failed to close completely. All proper lock out procedures were followed and leak tests and survey data revealed no elevated radiation levels. A service provider has been contacted for repairs. | ENS 50727 | 8 January 2015 05:00:00 | Agreement State | Agreement State Report - Gauge Failed to Function as Required | The following information was provided by the Commonwealth of Pennsylvania via email: During a daily routine maintenance inspection (at the Edgar Thomson Plant in Braddock, PA), the licensee observed that the Berthold moisture analyzer gauge was at an odd angle when compared to the chute that it was attached. A mounting bracket had broken causing the gauge to become dislodged. An area survey check was performed and determined no elevated radiation readings. The shutter was placed into the closed position and a service provider was notified. The gauge was then removed by the service provider and transported to their warehouse to determine the repair method. The licensee is also awaiting a response from the manufacturer. Manufacturer: Berthold Technologies, Isotope: Am-241, Activity: 300 mCi CAUSE OF THE EVENT: The bracket had broken allowing the device to shift. The device was still in place but not completely flush with the chute. ACTIONS: The device was removed from the site without incident and transported to the service provider's warehouse for repair. | ENS 47605 | 17 January 2012 13:00:00 | Agreement State | Agreement State Report - Broken Thermo Measure Tech Model 5204 Collimator | The following report was received via facsimile from the PA Department of Radiation Protection. Notifications: The department's western regional office received a phone call on January 17, 2012 and then referred the matter to the central office via email and telephone on January 18, 2012. This event is reportable within 24 hours under 10 CFR 30.50(b)(2)(i). Event Description: A collimator failure was discovered during a monthly routine maintenance check on January 17, 2012. The collimator had broken off of the shutter mechanism. The device is identified as - Manufacturer: Thermo Measure Tech; Model 5204; Isotope: Cs-137; Activity: 4 Ci. CAUSE OF THE EVENT: Equipment Failure ACTIONS: Upon discovery, a service provider was contacted and repairs were made by replacing the collimator with a spare one. Surveys were performed to verify appropriate conditions. No employees were exposed to excess levels of radiation as a result of this failure. A 30 day licensee report is expected.
