ENS 4956820 November 2013 16:25:00Agreement StateAgreement State Report - Troxler Gauge Involved in a FireThe State of Florida received a call from their licensee regarding a pick-up truck fire with a soil moisture density gauge in the back. On 11/20/13 at approximately 11:25 am EST on I-95 southbound near mile marker 169, a 2002 Ford F150 owned by the licensee caught fire. The exact cause of the fire is unknown. The fire spread so quickly, the licensee was unable to remove the Troxler Model 3440 portable density gauge (S/N: 77-4969) from the truck bed. The licensee notified emergency responders who arrived within 15 minutes and extinguished the fire. The gauge transportation container was damaged in the fire but there was no breach. The licensee RSO responded to the scene to conduct surveys. All surveys of the area were at or near background levels. Surveys of the gauge container and gauge were normal. The gauge itself showed no signs of damage from the fire. The licensee plans on installing the gauge into a spare transportation container to return it to the manufacturer for inspection and repair if necessary. The licensee will instruct employees to avoid driving through high grass and underbrush to avoid a similar incident in the future.

At approximately 1300 hours on 2/14/04, the technician returned home for lunch in the company truck with a Troxler Model 3430, S/N 30167, gauge containing two sources, i.e., 8 millicuries Cs-137 and 40 millicuries Am-241/Be, locked in its case in the truck bed. The chain securing the case was cut and the Troxler stolen. The technician has the key which locks the source in the retracted position. The Miami Police Department and the Florida State Warning Point were notified. The company will issue a press release and reward for the return of the gauge. Florida Incident No. FL04-116.

The Troxler gauge was found next to a garbage dumpster located on a grocery store parking lot. The gauge was still in its case and had not been damaged. Miami HAZMAT came to the scene and took radiation readings of the gauge in its case. Radiation readings were 2 millirem/hr (expected value). The gauge has been returned to its assigned owner.
