ENS 5194617 May 2016 22:30:0010 CFR 20.2201(a)(1)(i)Lost Am-241 SourceA B-52 bomber crashed shortly after take-off at Andersen AF base in Guam at 0830 local time (2230 GMT, Wednesday, 18 May 2016). The B-52 had a Sniper Pod which contained 12 microcuries of Am-241. So far the safety team could not locate the Sniper Pod at the incident spot. The value for Am-241 listed in Appendix C to (10 CFR) Part 20 is 0.001 microcuries. Each Sniper Pod carries 12 microcuries of Am-241. Hence, we (USAF) reported the incident to the NRC Operations Center in accordance with 10 CFR 20.2201. We will provide you (NRC with a written) incident report within 30 days. THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. ENS 5123515 July 2015 19:15:0010 CFR 20.2201(a)(1)(i)Lost Am-241 SourcesThe following was received via email: The Installation Radiation Safety Officer of the Wright- Patterson AFB telephoned at 1515 (EDT) stating that they lost two sources which have 10 microcuries of Am-241 each. IAW 10 CFR 20.2201, (the licensee has) reported the loss of sources to the NRC Operation Center. ENS 4819315 August 2012 04:00:0010 CFR 20.2201(a)(1)(i)Potentially Lost Americium-241 Source

The potentially lost source is a M43A1 Chemical Agent Detector Cell (Americium-241, 0.25 mCi). The source exceeds 1000 times the quantity listed in Appendix C to Part 20.

The source was made ready for shipment to our recycling and disposal office and given to the local traffic management office. Currently, we have no record of the shipment and cannot locate the shipping package. A comprehensive physical and document search is under way. Device Serial # Z03-C-30470 Source Serial # Z03-C-31725" The licensee notified R4 (Cook).

The following information was received by email from the licensee: The licensee received a call from the permit RSO (Radiation Safety Officer) that the Am-241 source has been recovered. It was located in the traffic management office. Notified the R1DO(Powell), R4DO(Hagar) and FSME via email. THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. ENS 4730930 September 2011 18:20:0010 CFR 20.2201(a)(1)(i)Lost Americium-241 SourceDuring a visual inspection of a source storage area at the Wright-Patterson AFB, 88th Medical Group, the licensee was unable to locate a 12 mCi Am-241 source. A rad survey was conducted but the source was not located. A search of the nuclear medicine area was performed with negative results. The source was last leak tested in July, 2002, then placed in storage. The last visual inspection where the source was identified was performed in September, 2009. The source was slated for disposal. The licensee is searching through their records to determine if the source, in fact, was sent to a disposal facility. The licensee notified Jackie Cook, R4DNMS.
