ENS 561317 September 2022 05:30:00Agreement StateAgreement State - Stolen and Recovered Troxler GaugeThe following information was provided by the California Department of Public Health (CDPH) via email: Twining Inc. reported the theft of a Troxler model 3430 # 30952 portable soils gauge to the Sacramento CDPH office via California OES (Office of Emergency Services) on 9/8/22. The gauge contains two sealed sources: Cs-137, 0.30 GBq (8mCi) and Am-241/Be, 1.48 GBq (40mCi). The theft occurred around 2230 PDT on 9/6/2022, from the gauge operator's home. A police report was made to a deputy with the Riverside County Sheriff's office on the morning of 9/7/2022. The gauge had an airtag attached to it for tracking purposes. On 9/22/2022, Maurer Technical Services, who calibrates these types of soils gauges was contacted by a person who indicated he had this gauge. The caller's name, phone number and address in Riverside were obtained. Maurer Technical verified the gauge's serial number belonged to Twining, Inc. and their office was notified. Staff from Twining's Riverside office went to pick-up the gauge and it was transferred back to the Ventura office, where it was evaluated and leak tested on 9/25/2022 (found to be not leaking). The Riverside County Sheriff's office was informed that the gauge was recovered. Additionally, the Radiologic Health Branch was also notified that the stolen gauge had been recovered. On 9/29/2022, it was discovered that a supervisor at ICE RHB had received the initial notification from the CDPH duty officer, but due to competing demands, failed to forward the report to the RHB South office for follow-up action and reporting to the NRC. CA Event Number: 092222 THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. ENS 5558213 November 2021 08:00:00Agreement StateStolen Moisture Density GaugeThe following was received from the California Department of Public Health (CDPH) via email: On Monday, November 15, 2021, Twining, Inc. reported the theft of a CPN MC-1DR #MD0080857 containing sealed sources of Cs-137 (10 mCi) and Am-241 (50 mCi). The theft occurred after 1700 (PST) on Saturday November 13, 2021 at a temporary job storage site (Clipper's Stadium in Inglewood, CA). There was a locked perimeter fence and a guard onsite. The CPN nuclear gauge was locked in its transport case, locked inside a job box, which was chained to a larger Conex box on site. Other equipment was also stolen from the jobsite. A police report is being filed. Additional information has been requested by CDPH. ENS 5124921 July 2015 07:00:00Agreement StateAgreement State Report - Damaged TroxlerThe following was received from the State of California via Email: A CPN thin layer density gauge, serial #1228, was run over by a dump truck that was spreading sand at a paving project on Highway 12 at Terminous Road in San Joaquin County, CA. The source was not breached and remained intact. The gauge was taken to Pacific Nuclear and a leak test disclosed no removable contamination. The California Code of Regulations, Title 17, Section 30350.3 states that the licensee shall immediately notify the Department by telephone and subsequently, within 30 days, by confirmation letter if the licensee knows or has reason to believe that a sealed source has been ruptured. This incident is being reported to the NRC as information only. California 5010 number: 072115
ENS 5057017 October 2014 12:45:00Agreement StateAgreement State Report - Stolen Troxler Moisture/Density GaugeThe following information was provided by the State of California via email: Twining, Inc. notified RHB (California Radiation Health Branch) that at approximately 0545 (PDT on 10/17/2014), a Troxler 4640 moisture/density gauge, containing 10 mCi of Cs-137 and 50 mCi of Am-241, was stolen from an employee's vehicle while he was in his hotel room (located in Oakdale, CA).
