ENS 449194 March 2009 04:00:00Agreement StateAgreement State Report Involving a Missing General Licensed SourceThe following information was received from the Commonwealth of Pennsylvania via fax: PA DEP Bureau of Radiation Protection (BRP) was notified in writing, dated March 9, 2009, by the Purchasing / Customer representative of Say Plastics. The device has not been found but they will continue to search for it. To prevent future incidents, they have created a sign out sheet that is to be signed when a worker checks the device out and returns the device. The State has an inspection scheduled for March 19, 2009 and will continue to keep NRC informed of the status of our investigation. Event description: Lost a model P2021 Nuclecel Static Eliminator City, State: McSherrystown, PA Description: (Serial Number A2GB696) (Manufacturer NRD, LLC) Device was discovered missing on March 4, 2009. Eliminator containing Po-210 (originally 10 mCi; decayed to approximately 1.4 mCi) PA Event Report ID No: PA090012 THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
